During menstruation, the body releases chemicals called prostaglandins. These help the uterus contract to shed its lining. This process is normal.
However, when inflammation levels are higher or the body is under ongoing stress, these signals can become stronger and more painful than they need to be.
This is where many women begin to experience more intense cramping, pelvic pain, and fatigue during their period.
Your nervous system plays a major role in how pain is experienced.
When the body is under chronic stress, the nervous system shifts into a protective state. In this state the body becomes more sensitive to pain signals. Stress hormones can also increase inflammation and reduce the body’s ability to settle and recover between cycles.
Over time the body’s alarm system becomes louder.
Normal uterine contractions can feel intense. Muscles tighten more easily. Organs become more sensitive. Recovery between periods becomes harder.
Instead of settling after each cycle, the body stays on high alert.
When pain continues for years without effective support, the body often responds protectively.
Muscles in the pelvic floor, abdomen, hips and lower back may tighten. Connective tissue can lose mobility. The nervous system may become increasingly reactive.
This can create a cycle of:

Persistent pelvic pain may occur with conditions such as endometriosis, but bladder pain, bowel discomfort or pain with intimacy can also develop even when endometriosis is not present.
