Coccyx (tailbone) pain can be debilitating. We know. It can impact your ability to sit for long periods of time, particularly on hard surfaces, cause pain during bowel movements, sex and coughing or sneezing. The trigger for coccyx pain can be caused by many different factors, including but not limited to: a direct fall on your bottom; sitting for long periods on a hard chair; chronic constipation; and vaginal delivery. Tailbone pain may also be caused by muscular tension from the buttocks and pelvic floor, which can be increased by lower back, pelvis or hip pain, and periods of high stress. Other triggers and contributing factors include rapid weight loss, resulting in a loss of cushioning, and being overweight. Women are five times more likely to experience coccyx pain.
Our skilled pelvic physiotherapist Leisa has a special interest in treatment of coccyx (tailbone) pain and has completed further training in pelvic health and pelvic / hip dysfunction. During treatment she aims to treat the cause of the problem whilst relieving the symptoms. This is done using a range of hands-on therapies and modalities including dry needling and the Hesch method. Leisa utilises a bottom up and top down approach to assist in calming the nervous system, normalise muscle tone including the pelvic floor, and eliminate reflexive changes up the body.
Prior to commencing treatment, Leisa will complete a thorough initial assessment, which will help to identify potential causes for your coccyx pain. She may use our real time ultrasound services to assist with this, which allows us to see how your muscles contract and relax whilst performing a movement, activity or exercise. She may provide objective assessment of your painful tasks, including sitting, pelvic girdle, hip and your coccyx – both externally and internally if you feel comfortable (more on this below).
There are a lot of muscles and fascia in the pelvis and hips that attach directly and indirectly onto the coccyx, which can aggravate pain.
That’s why an assessment of your pelvis and hips can give us clues to which muscles are tense, which helps us develop the best Cairns coccyx (tailbone) pain treatment plan for your specific needs.
In addition, your pelvis / sacrum movement can also give us clues, for example, gripping with the lower buttocks muscles as a strategy to move the hip can cause gripping of the pelvic floor muscles (PFM). PFM releases will feel great but will be short lived unless we work out why the lower buttocks muscles are gripping (they are usually trying to do the job of some muscles that are weak).
Understanding what is causing your pain is vital for us to develop an effective Cairns coccyx (tailbone) pain treatment plan for you. There are lots of pelvic floor muscles and fascia that attach directly and indirectly onto the coccyx, which can aggravate pain. These are best felt via a rectal examination. The coccyx angle can also be felt internally via a rectal examination, to see if it’s curled under contributing to pain. Some information can be gained via a vaginal examination and externally, if you don’t feel comfortable.
A combination of internal and external releases of the pelvic floor muscles can really help to reduce coccyx pain. In fact, most patients say it feels fantastic to relax the tight muscles. If you don’t feel comfortable that is ok, there is still heaps we can do to help you.
