CHRONIC DISEASE MANAGEMENT CAIRNS

Cairns Chronic Disease Management (CDM) Plans are issued by GPs. To be eligible, your GP must identify that you have a chronic medical condition that has been, or is likely to be, present for 6 months or longer. CDM Plans are a Medicare Government Scheme through which, and under certain circumstances, patients can access physiotherapy and other Allied Health provider services, including osteopathy, at a subsidised rate.

CONDITIONS WE CAN TREAT UNDER CDM PLANS

Please note there is no specific ‘list’ of eligible conditions which can be treated under a Cairns Chronic Disease Management Plan. Thanks to our training and the diversity of skills within our team of Allied Health professionals, we can support a range of presenting conditions, in conjunction with your CDM Plan. This includes but is not limited to, asthma, stroke, diabetes and an array of musculoskeletal conditions. We provide comprehensive assessment and treatment, with personalised treatment plans.

Our team of Allied Health professionals at Wellness Embodied includes Cairns physiotherapists and occupational therapists to ensure a holistic approach to chronic disease management. Our varied skill set ensures we are best placed to provide advice and specific treatment and support for your condition, rehabilitation and/or injury prevention. We use a whole of person approach for all our patients, including those with a Chronic Disease Management Cairns Plan.

Cairns Chronic Disease Management Plans

Chronic Disease Management Plans Cairns can be utilised to subsidise treatments for a range of presenting conditions. This includes, but is not limited to, the following:

The Wellness Embodied team of Allied Health professionals, have high-level knowledge of the entire musculoskeletal system. We know pain can be a result of imbalance in many systems- muscle, joint, bone, ligament, tendon, nerve, fascia, dura, autonomic system and more. We are here to help.

How Does the CDM Plan Work?

Chronic Disease Management Plans Cairns, may be issued by your GP at any time of the year. They remain ‘open’ until you have used the 5 subsidised treatments available. This must be done in the same calendar year, the CDM Plan was issued. Under no circumstances can extra visits be added. Any unused services on 31 December roll over to the next year, however, will be counted in the allowable amount (5 visits) for the next year.

The 5 Chronic Disease Management Cairns Plan visits may be allocated to 1 Allied Health provider, or spread between several providers, you will need to coordinate this with your GP. Once all is confirmed, your GP will issue your Chronic Disease Management Plan Cairns accordingly.

Sometimes, the GP will send the plan on your behalf to our clinic; however, we do recommend bringing a copy along on your first appointment. Please note our team think outside the box and are committed to helping clients, get relief from chronic pain and dysfunction.

Please note, appointments at Wellness Embodied under a Chronic Disease Management Plan Cairns – (formerly known as Enhanced Primary Care) – are subsidised (and not free). The current Medicare rebate (as at October 2025) is $61.80 per appointment. If you are referred to Wellness Embodied in your Care Plan, we ask that you pay your account with us, at the time of consultation.

We will process your Medicare rebate on the same day, via our Practice Management Software, and you will receive the rebate directly to your bank account, providing you have your up to date bank details registered with Medicare . If you do not have an account linked, we can issue a receipt that will allow you to claim your rebate directly with Medicare. At time of writing, we do not Bulk Bill.
